Quarterly Reports

Agency Monitoring Form This is the form that we complete when we conduct your periodic (and unannounced) site visit, and is provided so our partners will know what we are looking at/for.

Refrigerator/Freezer Temperature Log Temperatures must be checked weekly for each refrigerator and freezer, and the log maintained until the end of the following year (2014 temperature logs must be maintained until January 1, 2016)

Sample Sign in Sheet (non-USDA) This form is not required, but will assist in the collection of the demographics necessary to complete the required quarterly reports.

Food Handler Safety Course This course must be current, and is valid for two years.
